Pre-Screening Interest Form

Thank you for your interest in JCLEA's Day or Night Academy program. This short form is not the full Academy application — it helps our staff confirm basic eligibility and collect your contact information so we can follow up with you about next steps.

After you submit this form, a staff member will contact you to talk about scheduling an interview and to get you set up with the full application. The application is completed online through our secure system — we'll email you a login and a temporary password. No paper packet or PDF required.

The application also requires copies of certain official documents (for example, your birth certificate, Social Security card, and driver's license). A complete list is included in the application itself, so it's a good idea to start gathering these now.
